He had shaved his neck every second day for fifteen years and had razor bumps for most of them. Two rounds of antibiotics from a walk-in clinic had done nothing, because pseudofolliculitis barbae is a mechanical problem before it is an infection: the cut hair curls back and re-enters the skin.
On Fitzpatrick VI skin the alexandrite wavelength is not an option. We patch tested, then treated exclusively with the 1064 nm Nd:YAG at conservative fluences — six sessions six weeks apart — and asked him to clip rather than shave the treated strip of neck between visits.
The photograph is at seven months. The bumps and ingrown hairs are largely resolved. The darker marks they left behind are fading but still visible, and we told him at the outset that those would take longer than the bumps and that laser does not erase them.
He kept his beard along the jaw. We treated the neck only, which is where the problem was.
Individual results vary. Deeper skin tones need the right wavelength and conservative settings, and a course rushed on the wrong wavelength is how deeper skin gets burned.
